Athletics Registration

To register for athletics, please complete the following by December 4th:

Step 1: Student-athletes are required to have a physical examination from a certified medical professional noting that they are able to participate in high school sports.

**Physicals are good for 13 months. Once a physical expires 13 months to the date it was completed, a student will be unable to participate until a new physical is turned in to the nurse and the Co-Athletic Directors are notified. If you are unable to schedule a visit with your PCP, please note that select urgent care facilities do offer these physicals as well.***

Step 2: Athletic Medical and Consent Form. Complete and deliver to NDA’s Main Office or email to athletics@nda-worc.org.

Step 3: Pre-participation Concussion Reporting form. Complete and deliver to NDA’s Main Office or email to athletics@nda-worc.org.

Step 4: NFHS Concussion Education Course. Parents and students must take part in concussion education through the National Federation of High School Sports. Every student-athlete and at least one parent must complete the respective course and should deliver the certificates of completion to NDA’s Main Office or email to athletics@nda-worc.org.
