Admissions Materials

If you are interested in applying to Notre Dame Academy and would like an admissions packet mailed to you, please call Admissions Director Mary Riordan at at 508-757-6200, ext. 229, or e-mail the Admissions Office. 

You can download application materials in PDF form and mail them to Mary Riordan, Admissions Director, Notre Dame Academy, 425 Salisbury Street, Worcester, MA 01609.  You will need to download the following:

1. Entrance Exam Registration
2. Application Form (Form contains essay form to be filled out by applicant describing why she wants to attend Notre Dame Academy.)
3. Two teachers recommendation forms (to be completed by teachers of your choice who should mail them directly to the Academy's Admissions Office.)
4. Transmittal Form for Grades  (to be submitted to your present school so your transcript may be sent to the Academy.)

You may also submit your Entrance Exam Registration, your Application, and your Essay on-line using the On-line Entrance Exam Form, the On-Line Application Form, and the Application Essay.  Please download two Teacher Recommendation Forms and the Transmittal Form and give them to the appropriate individuals to complete.

Please note that there is a $25.00 Entrance Exam fee and a separate $25.00 Application fee.
