• Entrance Exam Registration Form
• Recommendations from two teachers (recommendation form)
• An essay, written by the applicant, explaining why she wants to attend Notre Dame
• The applicant's official school transcript including any standardized test scores (transmittal form for school included in the brochure packet)
• The completed application form (included in the brochure packet)
• A $25.00 application fee
